Volume 4: Medical Conditions & Women’s Hair Loss

Women’s Hair Loss knowledge Center on Medical Conditions, Can thyroid disease cause hair loss?, Does Hashimoto's cause thinning hair?, Can autoimmune disease affect hair?, What is Alopecia Areata?, What is Female Pattern Hair Loss?, What is Telogen Effluvium?, Can diabetes cause hair loss?, Can lupus cause hair loss?, Does PCOS cause thinning hair?, Can anemia make my hair fall out?

Hair loss can sometimes be more than a cosmetic concern. For many women, changes in hair density, excessive shedding, or sudden hair loss may occur alongside an underlying medical condition affecting the body’s normal hair-growth cycle.

In Volume 4 of the Hair Loss Heroes Women’s Hair Loss Knowledge Center, we explore medical conditions commonly associated with female hair loss, including thyroid disease, Hashimoto’s thyroiditis, autoimmune disorders, alopecia areata, female pattern hair loss, telogen effluvium, diabetes, lupus, PCOS, and anemia.

One of the most important lessons we have learned from evaluating women with hair loss is that there is rarely just one “smoking gun.” A medical condition may be one contributor alongside hormonal changes, nutritional deficiencies, metabolic health, medications, stress, genetics, or inflammation.

Understanding these relationships can help women ask better questions, seek appropriate medical evaluation, and begin identifying the combination of factors potentially affecting their hair.

31. Can Thyroid Disease Cause Hair Loss?

Thyroid disorders can contribute to diffuse hair thinning and increased shedding because thyroid hormones help regulate metabolism and the normal hair-growth cycle. Both hypothyroidism and hyperthyroidism may disrupt that cycle, causing more follicles to enter the resting and shedding phases. Women may notice increased hair in the shower, a thinner ponytail, reduced density, or changes in hair texture. Thyroid-related hair loss may also occur alongside other contributing factors, including iron deficiency, hormonal changes, nutritional deficiencies, medications, or autoimmune disease. A comprehensive evaluation may therefore include thyroid testing as part of a broader investigation into the possible causes of hair loss.

It is an autoimmune condition in which the immune system attacks the thyroid, potentially reducing thyroid hormone production and disrupting the hair-growth cycle. Women may experience diffuse thinning, excessive shedding, dry or brittle hair, or slower growth. Because Hashimoto’s is autoimmune in nature, evaluating hair loss may involve looking beyond thyroid hormone levels alone. Iron status, Vitamin D, nutrition, medications, inflammation, hormonal changes, and other health factors may also be relevant. Hair thinning in a woman with Hashimoto’s should therefore be viewed as potentially multifactorial rather than automatically attributed to a single abnormal laboratory value.

Autoimmune diseases can affect hair when the immune system mistakenly targets hair follicles or when systemic inflammation, illness, nutritional changes, or treatment disrupts normal hair growth. Conditions associated with hair loss include alopecia areata, lupus, autoimmune thyroid disease, and several other inflammatory disorders. The pattern can vary considerably—from distinct bald patches to widespread thinning or excessive shedding. Medications used to manage autoimmune disease may sometimes contribute as well. Because several factors can occur simultaneously, identifying an autoimmune condition does not necessarily identify the only cause. A thorough evaluation should consider the woman’s medical history, symptoms, medications, nutritional status, hormones, laboratory findings, and pattern of hair loss.

It commonly produces sudden, smooth, round or oval areas of hair loss on the scalp, although eyebrows, eyelashes, and other body hair can also be affected. Some women experience limited patches, while others develop more extensive hair loss. The follicles are not necessarily permanently destroyed, which means regrowth can occur, although the condition can be unpredictable. Alopecia areata is different from typical female pattern hair loss and requires appropriate medical evaluation. Associated autoimmune conditions, family history, nutritional status, and other potential contributors may also deserve consideration when evaluating the complete clinical picture.

Rather than developing the classic receding hairline commonly seen in men, women frequently notice a widening part, reduced density across the top of the scalp, and a progressively smaller ponytail. Genetics, aging, follicular sensitivity to androgens, and hormonal changes may all contribute. The condition can progress gradually over many years. Importantly, having female pattern hair loss does not mean other causes should be ignored. Menopause, thyroid disorders, iron deficiency, nutritional issues, medications, stress, and other conditions can occur simultaneously and potentially accelerate thinning.

The shedding often becomes noticeable several weeks or months after a triggering event. Potential triggers include major illness, surgery, significant emotional or physical stress, rapid weight loss, nutritional deficiencies, hormonal changes, childbirth, certain medications, or metabolic disturbances. Women typically notice increased hair in the shower, brush, clothing, or around the home rather than isolated bald patches. Telogen effluvium is often temporary, but persistent shedding warrants evaluation because an ongoing trigger—or several overlapping triggers—may continue disrupting the normal hair-growth cycle.

Poorly controlled blood glucose can affect small blood vessels that deliver oxygen and nutrients throughout the body, potentially influencing the environment surrounding hair follicles. Diabetes may also coexist with thyroid disease, hormonal abnormalities, nutritional deficiencies, and other conditions associated with hair loss. Significant illness or metabolic stress can additionally trigger telogen effluvium, resulting in increased shedding. Hair loss should not be assumed to be caused by diabetes alone. When a woman with diabetes develops noticeable thinning, the broader clinical picture—including medications, nutrition, hormones, thyroid function, and metabolic health—may warrant evaluation.

Lupus can cause several different patterns of hair loss. Systemic inflammation may trigger diffuse shedding, while certain forms of cutaneous lupus can directly affect the scalp and potentially cause scarring hair loss. Some women notice overall thinning, fragile hairs around the hairline, or increased shedding during periods of increased disease activity. Medications, nutritional deficiencies, anemia, thyroid disorders, stress, and other health factors may contribute simultaneously. Because scarring and non-scarring forms of hair loss require different approaches, unexplained hair loss in someone with lupus deserves appropriate medical evaluation. Early identification of scalp inflammation is particularly important when there is redness, scaling, tenderness, lesions, or localized areas of hair loss.

Women may notice progressive thinning across the crown or widening of the part while sometimes simultaneously experiencing increased facial or body hair, acne, irregular menstrual cycles, or difficulty managing weight. Follicles that are genetically sensitive to androgens may gradually produce finer, shorter hairs. However, PCOS may be only part of the explanation. Thyroid disorders, iron deficiency, nutritional issues, stress, medications, and other hormonal factors can coexist. A comprehensive assessment can help identify the combination of factors potentially influencing hair growth.

Anemia—particularly iron-deficiency anemia—can be associated with increased shedding and diffuse hair thinning. Hair follicles are highly active structures that depend on adequate oxygen, nutrients, and iron to support normal growth. When iron availability or red blood cell capacity becomes inadequate, the body may prioritize essential physiological functions over hair production, potentially disrupting the growth cycle. Women with heavy menstrual bleeding, restrictive diets, gastrointestinal conditions, or other sources of blood loss may be particularly susceptible. Importantly, iron status is only one possible contributor. Thyroid function, hormones, medications, illness, stress, nutrition, and other metabolic factors should also be considered when investigating persistent female hair loss.
